Housing Stabilization

(HSS Services)   

 This service has three parts that can be used to help residents of Minnesota find and maintain stable housing.  


Consultation

  • A service to help create a person-centered plan for individuals without MA case management services.       
  • Individuals seeking housing stabilization services have two options:

 This service has three parts that can be used to help residents of Minnesota find and maintain stable housing.  


Consultation

  • A service to help create a person-centered plan for individuals without MA case management services.       
  • Individuals seeking housing stabilization services have two options: they can self-refer or receive support from a provider or case manager. If someone has a Targeted Case Manager or Waiver Case Manager, they may assist in accessing services, or the individual can independently contact a housing stabilization services provider to receive the support they need.


Transition

  •  Can be used to help a person plan and move into stable housing.
  • Transition services encompass evaluating housing preferences and needs, identifying available options, and guiding through the application process. Additionally, it facilitates the moving process and aid in overcoming housing barriers. 
  • Weekly in-person check-ins are conducted to provide reminders and resources for long-term housing stability. 
  • Emphasizing client-driven goals for stability and independence, staff members prioritize community engagement and relationship building skills.


Sustain 

  • Aims to support the individual as they maintain their housing.
  • Sustain aims to equip individuals with the skills and resources necessary to maintain stable housing. 
  • Particularly beneficial for low-income seniors, it provides 150 hours of support for routine tasks like grocery shopping, light housework, and accessing community services. 
  • This service can significantly enhance the self-sufficiency of independent individuals, enabling them to lead healthy and active lives.

Offers expertise in the service areas of Housing Stabilization, Integrated Community Supports, Adult Foster Care, and Community Residential Services in Minnesota. Our team of Staff have combined 11+ years of working in the Health Human Service field as a Licensor and Licensed Social Worker

  • Home and Community Base Services (HCBS)Consulting 

Offers expertise in the service areas of Housing Stabilization, Integrated Community Supports, Adult Foster Care, and Community Residential Services in Minnesota. Our team of Staff have combined 11+ years of working in the Health Human Service field as a Licensor and Licensed Social Worker.

  

Agency Administration

  • Program Evaluation: Assessing program effectiveness and efficiency.
  • Organizational Analysis: Providing recommendations for improvement or leadership development.
  • Leadership Education: Offering training for leaders and managers.
  • Onsite Audits: Conducting assessments at the physical location.
  • Continuous Management Support: Providing ongoing assistance as the business expands.  

  

County/State/Federal Regulation and Compliance

  • Training on county, state, and federal Minnesota HCBS licensing regulations.
  • Assessment of operational practices to ensure compliance with Minnesota regulations.
  • Creation of customized agency policies.
  • Completion of licensing applications for all HCBS services in Minnesota.


  Mock Surveys

  • Mock Surveys: Simulated evaluations to ensure compliance with state, county, and federal regulations.
  • Personnel File Review: Examination of personnel records for compliance.
  • Policy Review: Assessment of organizational policies.
  • Physical Office Evaluation: Inspection of office premises.
  • Home Visits: Assessing client environments.
  • Interviews: Conducting discussions with staff or clients.
  • Assistance with Plan of Correction: Support in developing and implementing corrective actions post-survey.

Step 1. Fill out Self Referral Form

 STEP 2. Get the PSN to the qualified professional to complete.  

  •   Qualified professional, like a mental health professional, nurse practitioner, or doctor
  • VCS can provide a PSN if needed.

STEP 3. Make a Person-Centered Plan

  •  If you have a MA-Waiver case manager, senior care coordinator, or targeted case m

  •   Qualified professional, like a mental health professional, nurse practitioner, or doctor
  • VCS can provide a PSN if needed.

STEP 3. Make a Person-Centered Plan

  •  If you have a MA-Waiver case manager, senior care coordinator, or targeted case manager, that person will help you update your plan to include Housing Stabilization Services.  
  •  If you don’t have any of these people helping you, you can get help making a Housing Focused Person-Centered Plan from a registered housing consultant. (which we can offer referrals for) No matter who helps you with your person-centered plan, you get to decide which provider will help you with housing transition and housing sustaining services.  

STEP 4. Get Approved… 

  • Your documents are sent to the Minnesota Department of Human Services (DHS) for approval.  
  • To be eligible for this service one must reside in MN, be 18 or older, on Medical Assistance, and a disabling condition verified with a professional statement of need (PSN). The PSN is a document that is completed by a professional which outlines what supports are needed. 
  • The recipient then completes a person-centered plan that puts them in control of how best to utilize the supports they will receive.
